School Nurse - Suzanne Grim - 631-665-5873 spsnurse@spsbayshore.org
Please call the health office if you have any questions or concerns, or if your child has any medical history, problems, or special needs that we should be aware of. Thank you.
Attendance:
Please call the school health office (631-665-5873) or send an email to spsnurse@spsbayshore.org whenever your child is absent from school. Upon return to school, all students must provide the teacher with a note stating the reason for absence even if a call has been made.
When is my child sick enough to stay home from school??
A child who is sick will not be able to perform well in school and is likely to spread the illness to other children. We suggest making a plan for childcare ahead of time so you will not be caught without a comforting place for your child to stay if he/she is ill. Please do not send your child to school if he/she has:
* Fever in the past 24 hours
* Vomiting or diarrhea in the past 24 hours
* Chills
* Sore throat
* Strep throat (must have taken antibiotics for at least 24 hours before returning)
* Bad cold, with a very runny nose or bad cough
* If your child needs medicine (ibuprofen, Tylenol, etc.) to feel well or be fever free
If your child becomes ill at school and the teacher or school nurse feel that he/she is too sick to benefit from school or is contagious to other children, you will be called to come and take him/her home from school.
It is essential that we have a phone number where you can be contacted during the day and emergency numbers in the event you cannot be reached. Please be sure that arrangements can be made to transport your child home from school and that childcare is available in case of illness.
If your daytime or emergency phone numbers change during the year, please notify the school immediately.
Medications:
If your child needs any medication (Tylenol, inhaler, EPI-PEN, etc.) in school on a regular or occasional basis, please contact the nurse to discuss the school's medication policy and to obtain the necessary form (available on the link above) for your doctor. If at any time your child needs to take medication during the school day for only a few days, (such as cough medicine or antibiotic), please bring a written request signed by both you and the doctor to the health office with the medication. All medications must be transported to and from school by a parent or other adult. Older students may carry and use their inhalers as needed for asthma only if I have written authorization from parent and physician. Students may not at any time use or have in their possession any medication or medical device without proper authorization. We must strictly enforce these rules for the health and safety of all of our students.
Physical Education Participation:
If your child is unable to participate in gym class, please send a note stating the restriction. If this restriction will be for longer than one week, a note from your doctor is required. Any student who is unable to participate in gym class will also be restricted from participating in outdoor recess.
Immunizations and Physicals:
All students must have proof of immunization. If your child's immunization record is incomplete, you will be notified and must provide evidence of the needed immunization to allow continued school attendance. New York State law requires all new entrants, and students in grades K, 1, 3, 5, and 7 to have a physical exam. If your child is in a grade requiring an exam please schedule one as soon as possible and have a copy of the exam sent to school (the form is available above). If record of a physical by your family doctor is not received, the school physician will examine your child.
Screenings:
Vision, hearing, and scoliosis screening will be done as required by New York State guidelines. Should there be a need for a more complete exam by your physician you will be notified.
